Multiple ICD Replacements can Predict Complications

Patients who have had previous pocket procedures are most at risk

A Canadian study has found that patients with implantable cardioverter defibrillators (ICDs) have an increased risk of suffering complications, particularly if they have had multiple, previous pocket procedures.

Conducted by Dr. Andrew Krahn and his colleagues at the London Health Sciences Center in Ontario, the study found a higher-than-expected overall ICD replacement complication rate.

The study included 451 patients who had ICD devices replaced between October 2004 and October 2005.The paients were monitored for both major and minor complications. Major complications include serious infections (those that require the ICD be removed), bleeding, or ICD malfunction. Minor complicatiions incude small infections and worsening of other medical condiions.

After one year, 41, or 9.1% of the patients experienced complications, according to the study, published in this month’s issue of HeartRhythm Journal. An additional 27 patients, or 5.9%, needed addtional surgery. Two patients died, and 14  (3.1%) experienced minor complications.

“No prior research has investigated the potential risk factors associated with ICD replacement, more specifically, what makes some patients more at-risk than others,” Krahn said in a news release. “Study findings will help physicians better understand complication risks for patients who may need ICD replacement, and ultimately, provide better care for their patients.”
